Rigging issues

When using my swim bait that Ive recently made , Ive noticed when working it in the water, my bait wants to bow out from the line, meaning the line isn't staying center with the bait, and in return the bait isn't wanting to swim right. I have it rigged, line through loop, hook embedded. Im not using a leader. The picture is kind of blurry, but hope it helps with what im trying to explain. any suggestions?

It looks like you're pretty close. I would try a couple of things: make the tail 50% smaller, try a little more softener, add shot to the belly to create a lower center of gravity, experiment with the hook location (usually the more forward, like the eye of the hook touching the underside of the nose ring, experiment with hook weight.
